The Germany international, who had been under contract at Stadio Olimpico until 2020, joins the Premier League champions on a five-year deal after impressing at the 2017 Confederations Cup.
"It's a great feeling because not every player has the opportunity to join a huge club like this," the 24-year-old defender told his new club's official website.
"I am looking forward to meeting up with my new team-mates and I'm very proud to officially become a Chelsea player."
Conte needed reinforcements at the back after allowing Branislav Ivanovic, John Terry and Nathan Ake to leave this year – the latter two at the end of the campaign.
